Medical weight loss
Weight is rarely just about willpower. Our provider-led program starts with a full medical evaluation, then a plan built around your health history, your goals, and your week, at our Westover Hills clinic in San Antonio or on Main Street in Boerne.
Everything in the program is handled in-house by the same team, from your first assessment to your follow-up scans.
The program
A medically supervised program designed to help adults in San Antonio and Boerne lose weight safely with a personalized plan. Our goal is simple: help you lose weight, improve your health, and keep it off.
Prescription options
Both work with your body’s own appetite signals. Your provider will recommend one only if it is right for you.
Acts on two gut hormone receptors to reduce appetite, help you feel full sooner and improve how your body handles blood sugar.
A small injection under the skin.
Increased gradually as you tolerate it.
Adults with obesity, or overweight with a weight-related condition.
Mimics a natural gut hormone to reduce appetite, slow digestion and help you feel full longer.
A small injection under the skin.
Over several weeks to limit side effects.
Adults with obesity, or overweight with a weight-related condition.
Both need a prescription and a medical evaluation. Common side effects include nausea and digestive upset, especially early on. Not suitable for everyone, including people with certain thyroid or pancreatic conditions.
Weekly support
Many patients add weekly Lipolean and vitamin B12 injections to their program. Given in-office during your weekly visit, they’re a quick add-on your provider may recommend alongside nutrition changes and, when appropriate, medication.
Injections support your plan; they aren’t a substitute for it. Your provider will tell you whether they’re a good fit.

What to expect
From your first visit to long-term maintenance, your plan is run by one primary care team at Westover Hills or Boerne. No hand-offs, no anonymous online prescribers. Here’s what each stage looks like.
We start with your full medical history, current medications, past weight loss attempts, and goals. Your provider orders a baseline blood panel (BMP, CBC, TSH, lipid panel, and A1c), with a full wellness panel added if needed, so your plan starts from your numbers, not guesswork. Your first InBody scan records your starting muscle, fat, and visceral fat.
Bring: photo ID, insurance card, and a list of medications and supplements.
Your provider reviews your results with you and builds a plan around your health, schedule, and preferences. That may include nutrition and activity guidance, semaglutide or tirzepatide if you qualify, and weekly lipotropic and B12 injections. If medication is part of your plan, we show you how to give your first weekly injection.
Quick weekly check-ins for your lipotropic and B12 injections, plus a weight and side-effect check. Many patients say these short visits are what keep them on track.
A repeat InBody scan shows exactly what you’re losing, so we can confirm it’s fat and not muscle, and adjust protein, activity, or medication if needed.
Your provider reviews your progress, InBody results, and side effects, then fine-tunes your plan. If you’re on semaglutide or tirzepatide, this is when dose increases are decided, one step at a time.
Reaching your goal isn’t the end. We shift to a maintenance plan with less frequent visits, nutrition and strength habits that protect your results, and, if you’re on medication, a careful plan to stay at a maintenance dose or taper safely. Learn about weight management →

Alamo Primary Care
We are thrilled with our first visit here. Everything moved very quickly and efficiently, as they seem to have a very effective team of employees. The appointment lasted approximately 1 hour, and that included new patient intake forms, vitals, seeing the doctor and getting labs done. I love how fast the process went, while also feeling seen, heard and feeling like our concerns were valid and taken seriously. The receptionists were all very friendly, inviting and helpful, including Delilah, who checked us out and set our next appointment. Finding a new primary care provider can be quite overwhelming and time consuming, but not here, and the services offered go above and beyond anything we expected. We look forward to future visits with this practice.
Alamo Primary Care has the BEST staff! From the moment I walked in, I had such a great experience as a new patient. I’m especially thankful they were able to get me in for a same day appointment. Im finally feeling like I’m getting the answers I’ve been looking for. Highly recommend!
We came in to establish care with my new PCP, Dr. Vishal, and his staff were absolutely wonderful! Everyone was so informative, patient, and thorough throughout the entire process. I’m so happy that I made the change and feel confident knowing I’m in such great hands. Thank you, Dr. Vishal and team, for making us feel so welcomed and cared for!
I’ve had a great experience with Alamo Primary Care. Everyone in the office has always been friendly, welcoming, and willing to help. The front desk ladies are always kind and make everything easy, whether it’s scheduling appointments or answering questions. The doctor takes the time to listen and explain things without making you feel rushed, and the PA has been just as helpful and knowledgeable. It’s nice to go to a doctor’s office where you feel like they actually care about their patients. Highly recommend!
My senior mother had an appointment there today and let me know how well every member of your staff treated her from beginning to end. I appreciate your professionalism, empathy and amazing bedside manner. I’m happy to know she felt comfortable and in good hands. Highly recommend! Respectfully, patients daughter.
I had an excellent experience with this doctor and their entire office staff. From the moment I walked in, I was greeted warmly and treated with kindness and professionalism. The front desk team was efficient and helpful, making the check-in process smooth and stress-free. The doctor took the time to listen carefully to my concerns, explained everything clearly, and made sure I felt comfortable with the treatment plan. I never felt rushed, and it was clear that they genuinely care about their patients’ well-being.
I had a very positive experience at this primary care office. The staff was friendly, professional, and made me feel comfortable from the moment I walked in. Dr. Vishal Nemargommula took time to listen carefully to my concerns, answered my questions thoroughly, and never made me feel rushed. He was very kind, professional and easy to talk to. I especially appreciated how respectful and understanding everyone was during my visit. The office was clean, organized, and the overall atmosphere was welcoming.
This was my first time at this location and I was pleasantly surprised at every turn of this appointment. The front staff were incredibly welcoming and equally as funny. I’m a nervous talker so they humored me and it helped me calm down a lot. The doctor was punctual and very thorough for a first time visit which was a pleasant surprise. He had several follow up questions about each of my concerns and gathered more information as well as educated me more than any physician I’ve seen on a first visit. Excellent first impression and visit.
I cannot say enough good things about my experience with Dr. Vishal. From the moment I walked in, I felt comfortable from the check in with the ladies up front to the medical assistants in the back! He talked to me, not at me. He listened to my concerns, asked thoughtful questions, and made me feel validated. He came highly recommended and I never felt one ounce of regret. It’s such a relief to finally have a primary care doctor who makes you feel seen and heard. 10/10 recommend!
Very nice customer service it was a very pleasant visit of what I was dreading to be bad news. Every single nurse had a great attitude and made me feel very comfortable and welcomed. I’m glad I found this place and it’s only 5 minutes away from me. Plus they do IV therapy!!! Also the receptionist were awesome Andrea, Jen, Delilah were so nice to my kids they gave them Christmas light up necklaces because my kids asked for stickers. Great people

Alamo Primary Care prescribes semaglutide at its Westover Hills clinic in San Antonio and its Main Street clinic in Boerne. Providers prescribe it only after a full medical evaluation and labs, then manage dosing and monitor side effects at regular follow-ups.
Alamo Primary Care offers tirzepatide for weight loss at its San Antonio and Boerne clinics. Tirzepatide is a once-weekly dual GIP and GLP-1 injection, started at a low dose and increased gradually, and is prescribed only after a provider reviews your history and lab results.
Semaglutide acts on one appetite hormone receptor (GLP-1), while tirzepatide acts on two (GIP and GLP-1). Both are once-weekly injections that reduce appetite and help you feel full sooner. At Alamo Primary Care, your provider recommends the option that best fits your health history and goals.
Semaglutide and tirzepatide are generally considered for adults with obesity, or adults who are overweight with a weight-related condition such as high blood pressure, prediabetes, or high cholesterol. People with certain thyroid or pancreatic conditions may not qualify. Alamo Primary Care confirms eligibility through labs and a medical evaluation.
Yes. Primary care providers can prescribe weight loss medications such as semaglutide and tirzepatide after a medical evaluation. At Alamo Primary Care, the same team that knows your health history orders your labs, prescribes treatment, and tracks your progress with monthly visits.
Most patients start with a basic metabolic panel (BMP), complete blood count (CBC), thyroid test (TSH), lipid panel, and A1c. Alamo Primary Care may add a full wellness panel, including vitamin D, B12, iron, hormone, kidney, and liver tests, based on your history.
The most common side effects are nausea, digestive upset, and reduced appetite, especially in the first weeks or after a dose increase. Starting at a low dose and increasing gradually helps limit them. Alamo Primary Care monitors side effects closely at every follow-up visit.
An InBody scan measures muscle, body fat, and water separately in about one minute while you stand on a platform. Alamo Primary Care uses repeat InBody scans in San Antonio and Boerne to make sure patients are losing fat, not muscle.
At Alamo Primary Care, patients typically visit weekly for B12 and Lipolean injections and monthly for a provider follow-up. Monthly visits include progress review, InBody results (every other month), and medication dose adjustments when needed.
