Losing weight is one step. Keeping it off is the goal. Our weight management program at Alamo Primary Care focuses on the everyday habits, nutrition, and health conditions that shape your weight over the long term, with a primary care team that knows your history and stays with you.
Weight loss is about reaching a lower weight. Weight management is about everything that keeps you there: how you eat, move, sleep, and handle stress, and how conditions like diabetes, thyroid problems, or hormone changes affect your body. Our program covers both, but puts the long game first.
Weight loss program: a medically supervised plan that may include semaglutide or tirzepatide.
Weight management program: a sustainable plan for nutrition, activity, habits, and metabolic health, with or without medication.

We look at labs that affect weight, including thyroid, A1c, cholesterol, vitamin D, and hormone levels, so we’re treating the cause, not just the number on the scale.
Practical eating plans built around foods you actually enjoy, your schedule, and your budget. No crash diets, no rigid meal plans you can’t keep.
Realistic movement goals based on your fitness level, joint health, and time, whether you’re starting from zero or getting back into routine.
Support for the patterns behind weight gain: stress eating, late-night snacking, poor sleep, and all-or-nothing thinking.
Some common medications can cause weight gain. We review what you take and adjust where it’s safe to do so.
Regular check-ins and InBody body composition scans show changes in fat and muscle, so you can see progress even when the scale stalls.
If you’ve used semaglutide or tirzepatide, we help you taper safely and build the habits that protect your results long term.
Full history, current medications, and baseline labs.
We agree on realistic targets for weight, health markers, and daily habits.
Nutrition, activity, and behavior steps tailored to you.
Review progress, InBody results, and lab trends, then adjust.
Less frequent visits once you’ve reached your goal, with support whenever you need it.
Weight and chronic disease are closely linked. Losing even 5–10% of body weight can improve blood sugar, blood pressure, and cholesterol. Because we’re your primary care team, your weight plan is coordinated with your diabetes, heart health, and thyroid care, not handled separately.

Alamo Primary Care
We are thrilled with our first visit here. Everything moved very quickly and efficiently, as they seem to have a very effective team of employees. The appointment lasted approximately 1 hour, and that included new patient intake forms, vitals, seeing the doctor and getting labs done. I love how fast the process went, while also feeling seen, heard and feeling like our concerns were valid and taken seriously. The receptionists were all very friendly, inviting and helpful, including Delilah, who checked us out and set our next appointment. Finding a new primary care provider can be quite overwhelming and time consuming, but not here, and the services offered go above and beyond anything we expected. We look forward to future visits with this practice.

We came in to establish care with my new PCP, Dr. Vishal, and his staff were absolutely wonderful! Everyone was so informative, patient, and thorough throughout the entire process. I’m so happy that I made the change and feel confident knowing I’m in such great hands. Thank you, Dr. Vishal and team, for making us feel so welcomed and cared for!
I’ve had a great experience with Alamo Primary Care. Everyone in the office has always been friendly, welcoming, and willing to help. The front desk ladies are always kind and make everything easy, whether it’s scheduling appointments or answering questions. The doctor takes the time to listen and explain things without making you feel rushed, and the PA has been just as helpful and knowledgeable. It’s nice to go to a doctor’s office where you feel like they actually care about their patients. Highly recommend!
My senior mother had an appointment there today and let me know how well every member of your staff treated her from beginning to end. I appreciate your professionalism, empathy and amazing bedside manner. I’m happy to know she felt comfortable and in good hands. Highly recommend! Respectfully, patients daughter.
I had an excellent experience with this doctor and their entire office staff. From the moment I walked in, I was greeted warmly and treated with kindness and professionalism. The front desk team was efficient and helpful, making the check-in process smooth and stress-free. The doctor took the time to listen carefully to my concerns, explained everything clearly, and made sure I felt comfortable with the treatment plan. I never felt rushed, and it was clear that they genuinely care about their patients’ well-being.
I had a very positive experience at this primary care office. The staff was friendly, professional, and made me feel comfortable from the moment I walked in. Dr. Vishal Nemargommula took time to listen carefully to my concerns, answered my questions thoroughly, and never made me feel rushed. He was very kind, professional and easy to talk to. I especially appreciated how respectful and understanding everyone was during my visit. The office was clean, organized, and the overall atmosphere was welcoming.
This was my first time at this location and I was pleasantly surprised at every turn of this appointment. The front staff were incredibly welcoming and equally as funny. I’m a nervous talker so they humored me and it helped me calm down a lot. The doctor was punctual and very thorough for a first time visit which was a pleasant surprise. He had several follow up questions about each of my concerns and gathered more information as well as educated me more than any physician I’ve seen on a first visit. Excellent first impression and visit.
I cannot say enough good things about my experience with Dr. Vishal. From the moment I walked in, I felt comfortable from the check in with the ladies up front to the medical assistants in the back! He talked to me, not at me. He listened to my concerns, asked thoughtful questions, and made me feel validated. He came highly recommended and I never felt one ounce of regret. It’s such a relief to finally have a primary care doctor who makes you feel seen and heard. 10/10 recommend!
Very nice customer service it was a very pleasant visit of what I was dreading to be bad news. Every single nurse had a great attitude and made me feel very comfortable and welcomed. I’m glad I found this place and it’s only 5 minutes away from me. Plus they do IV therapy!!! Also the receptionist were awesome Andrea, Jen, Delilah were so nice to my kids they gave them Christmas light up necklaces because my kids asked for stickers. Great people
