What Are Lipotropic Injections?
Lipotropic injections, often called lipo shots or lipo B12 injections, are blends of nutrients involved in how the body processes and uses fat. Our lipotropic injection, Lipolean, typically contains:
- Methionine: an essential amino acid that supports fat metabolism in the liver
- Inositol: a vitamin-like compound involved in breaking down fats
- Choline: a nutrient that helps transport fat out of the liver
- B vitamins: support energy production and normal metabolism
Together, methionine, inositol, and choline are known as MIC, which is why these shots are also called MIC B12 injections.

What Are B12 Shots?
Vitamin B12 is essential for energy, red blood cell production, and nerve health. A B12 shot delivers it directly into the muscle, bypassing the digestive system, which makes it especially useful for people who don’t absorb B12 well from food or supplements.
Who May Benefit from B12 Shots?
You may be at higher risk of low B12 if you:
- Take metformin for diabetes or prediabetes
- Take acid-reducing medications long term
- Are on a GLP-1 medication like semaglutide or tirzepatide and eating significantly less
- Follow a vegetarian or vegan diet
- Are over 50, when B12 absorption naturally declines
- Have had weight loss surgery or digestive conditions
Your provider can check your B12 level as part of your lab panel.
How Lipo B12 Injections Fit Into Your Weight Loss Plan
Lipotropic and B12 injections work best as support, not a shortcut. Patients most often use them alongside:
- Nutrition and activity changes from their weight management plan
- Semaglutide or tirzepatide, when prescribed
- Monthly follow-ups and InBody scans to track fat and muscle changes
Many patients say weekly injection visits help them stay accountable and motivated. Evidence for lipotropic injections on their own is limited, so we use them as one part of a complete, medically guided plan.

Alamo Primary Care
We are thrilled with our first visit here. Everything moved very quickly and efficiently, as they seem to have a very effective team of employees. The appointment lasted approximately 1 hour, and that included new patient intake forms, vitals, seeing the doctor and getting labs done. I love how fast the process went, while also feeling seen, heard and feeling like our concerns were valid and taken seriously. The receptionists were all very friendly, inviting and helpful, including Delilah, who checked us out and set our next appointment. Finding a new primary care provider can be quite overwhelming and time consuming, but not here, and the services offered go above and beyond anything we expected. We look forward to future visits with this practice.

We came in to establish care with my new PCP, Dr. Vishal, and his staff were absolutely wonderful! Everyone was so informative, patient, and thorough throughout the entire process. I’m so happy that I made the change and feel confident knowing I’m in such great hands. Thank you, Dr. Vishal and team, for making us feel so welcomed and cared for!
I’ve had a great experience with Alamo Primary Care. Everyone in the office has always been friendly, welcoming, and willing to help. The front desk ladies are always kind and make everything easy, whether it’s scheduling appointments or answering questions. The doctor takes the time to listen and explain things without making you feel rushed, and the PA has been just as helpful and knowledgeable. It’s nice to go to a doctor’s office where you feel like they actually care about their patients. Highly recommend!
My senior mother had an appointment there today and let me know how well every member of your staff treated her from beginning to end. I appreciate your professionalism, empathy and amazing bedside manner. I’m happy to know she felt comfortable and in good hands. Highly recommend! Respectfully, patients daughter.
I had an excellent experience with this doctor and their entire office staff. From the moment I walked in, I was greeted warmly and treated with kindness and professionalism. The front desk team was efficient and helpful, making the check-in process smooth and stress-free. The doctor took the time to listen carefully to my concerns, explained everything clearly, and made sure I felt comfortable with the treatment plan. I never felt rushed, and it was clear that they genuinely care about their patients’ well-being.
I had a very positive experience at this primary care office. The staff was friendly, professional, and made me feel comfortable from the moment I walked in. Dr. Vishal Nemargommula took time to listen carefully to my concerns, answered my questions thoroughly, and never made me feel rushed. He was very kind, professional and easy to talk to. I especially appreciated how respectful and understanding everyone was during my visit. The office was clean, organized, and the overall atmosphere was welcoming.
This was my first time at this location and I was pleasantly surprised at every turn of this appointment. The front staff were incredibly welcoming and equally as funny. I’m a nervous talker so they humored me and it helped me calm down a lot. The doctor was punctual and very thorough for a first time visit which was a pleasant surprise. He had several follow up questions about each of my concerns and gathered more information as well as educated me more than any physician I’ve seen on a first visit. Excellent first impression and visit.
I cannot say enough good things about my experience with Dr. Vishal. From the moment I walked in, I felt comfortable from the check in with the ladies up front to the medical assistants in the back! He talked to me, not at me. He listened to my concerns, asked thoughtful questions, and made me feel validated. He came highly recommended and I never felt one ounce of regret. It’s such a relief to finally have a primary care doctor who makes you feel seen and heard. 10/10 recommend!
Very nice customer service it was a very pleasant visit of what I was dreading to be bad news. Every single nurse had a great attitude and made me feel very comfortable and welcomed. I’m glad I found this place and it’s only 5 minutes away from me. Plus they do IV therapy!!! Also the receptionist were awesome Andrea, Jen, Delilah were so nice to my kids they gave them Christmas light up necklaces because my kids asked for stickers. Great people

What is in a lipotropic injection?
Lipotropic injections typically combine methionine, inositol, and choline (MIC) with B vitamins such as B12. These nutrients are involved in fat metabolism and energy production. Alamo Primary Care’s lipotropic injection, Lipolean, is used as part of a medically supervised weight loss plan.
What is the difference between lipo B12 and regular B12 shots?
A regular B12 shot contains only vitamin B12, which supports energy and nerve health. A lipo B12 shot adds lipotropic nutrients like methionine, inositol, and choline that are involved in fat metabolism. Alamo Primary Care offers both in San Antonio and Boerne.
What are the benefits of B12 shots?
B12 shots can raise low B12 levels quickly, which may improve fatigue, low energy, and symptoms such as tingling or poor concentration linked to deficiency. They are especially helpful for people who absorb B12 poorly, including those taking metformin or over age 50.
How often should you get lipo B12 injections?
Most patients at Alamo Primary Care receive lipotropic and B12 injections once a week during their weight loss program.
Should I take B12 while on semaglutide or tirzepatide?
People taking GLP-1 medications often eat much less, which can lower intake of B12 and other nutrients. Alamo Primary Care checks B12 levels for San Antonio and Boerne patients on semaglutide or tirzepatide and may recommend B12 shots if levels are low.
Do lipotropic and B12 injections hurt?
Most people feel only a quick pinch. The injection takes seconds, and mild soreness at the injection site, if any, usually fades within a day.
Are Lipolean and B12 shots cash pay?
They are offered as a cash pay service at Alamo Primary Care.
